  • 4:30 PM 40 Hours per Week Occasional weekend and holiday coverage required Position Overview "Navitas Healthcare, LLC" is seeking an experienced Physical Therapist for a 13-week rehabilitation assignment in an outpatient and home health setting.
This role focuses on evaluating patients, developing individualized treatment plans, and delivering evidence-based physical therapy interventions to support functional recovery and independence. The ideal candidate will demonstrate strong clinical judgment, independent working ability, and experience coordinating care within interdisciplinary healthcare teams. Key Responsibilities Evaluate new patients and establish functional treatment goals Develop and implement individualized physical therapy treatment plans Perform musculoskeletal, neurological, and functional assessments Administer therapeutic modalities and adjust care plans based on progress Reassess patients and document clinical outcomes regularly Provide patient and caregiver education on exercises and recovery plans Coordinate care and communicate progress with physicians and care teams Maintain accurate and timely clinical documentation Supervise Physical Therapist Assistants, aides, and students as needed Participate in quality improvement and departmental initiatives Ensure compliance with safety, infection control, and regulatory standards Support staff training and clinical development activities Maintain daily patient activity and billing documentation Requirements Bachelor's degree in Physical Therapy from an accredited program Active Physical Therapy license in Massachusetts Current BLS certification required Strong communication skills in English Ability to work independently in community-based care settings For more details contact at or Call / Text at 516-862-1169.
