  • Full-Time LTC Therapy:
    Holyoke Holyoke, MA $45
  • 50/hour LTC Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team in Holyoke, MA.
This role offers the opportunity to work in a skilled nursing setting focused on helping residents improve strength, mobility, balance, and overall functional independence.
Position Responsibilities:
Complete physical therapy evaluations and develop individualized treatment plans Provide therapy focused on gait training, transfers, strengthening, balance, endurance, and mobility goals Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to support resident care plans Monitor resident progress and update treatment approaches as appropriate Maintain accurate and timely documentation in accordance with facility standards
