Job Description

Registered Physical Therapist
LOCATION
Salmon Homecare and Hospice 37 Birch Street Milford, MA 01757, USA We are seeking a compassionate and skilled Full Time Physical Therapist (PT). The ideal candidate will organize and conduct medically prescribed physical therapy programs to restore function, improve mobility, and prevent disabilities following disease, injury, or surgery. This position plays a vital role in helping patients achieve their maximum functional independence and quality of life. Key Responsibilities Provide physical therapy care for adult and geriatric patients (ages 18+). Perform patient evaluations and establish individualized therapy programs as prescribed by a physician. Apply a variety of therapeutic techniques including exercise, mobility training, massage, heat, water, and electrical stimulation. Conduct diagnostic and prognostic tests of muscle, nerve, joint, and functional abilities. Supervise and delegate duties to Physical Therapy Assistants (PTAs), ensuring high-quality patient care. Guide patients through active and passive exercises, gait training, and functional mobility activities. Educate patients and families on posture control, safe transfers, and exercises to continue at home. Instruct patients on the use of assistive devices such as braces, crutches, canes, and wheelchairs. Adapt treatment plans and techniques to meet patients' unique physical and communication needs. Maintain accurate and timely documentation of patient progress and therapy sessions. Participate in interdisciplinary discharge planning to ensure safe and effective transitions of care. Attend staff meetings, in-services, and ongoing training sessions. Comply with hospital safety, infection control, and compliance policies.
Qualifications Education & Experience:
Bachelor's degree required; Master's preferred. Experience in hospital or rehabilitation settings preferred.
Licensure & Certifications:
Current Physical Therapist (PT) license in the state of employment. Basic Life Support (BLS) certification required within 30 days of hire and maintained throughout employment.
Skills:
Strong communication and documentation skills. Ability to work collaboratively with interdisciplinary teams. Basic computer and word processing proficiency.
Pay:
$50.00 - $54.00 per hour
