Amergis Physical Therapist | Home Health - Danvers, MA Southborough, MA Apply Amergis Healthcare Staffing is partnered with a much-respected facility in Danvers, MA who is currently seeking a qualified Home Health Physical Therapist to assist them on a contractual basis.

ASAP Duration:

13 weeks - likelihood of extension

Schedule:

40 hours per week 8a-4:30p

M-F Contract Requirements:

Prior home health experience Prior Oasis and Homecare Homebase experience Reliable transportation during assignment Must be licensed driver with an automobile that ensured in accordance with state/or organization requirements and is in good working order Must complete documentation after each visit and transmit documentation at end of day PT's will have a Start of Care (SOC) each day and 4 visits, at a minimum, if time allows additional visits could be added - PT's case manage

MA PT License Pay:

$2,880/week local $3,199/week travel
