Job Description

Tender Touch Rehab Services is seeking a compassionate Physical Therapist Floater to provide high-quality, patient-centered rehab services across our skilled nursing and long-term care facilities. In this role, you will deliver evaluations, develop individualized treatment plans, and provide evidence-based interventions to improve seniors' mobility, function, and quality of life. As a floater, you will flexibly support multiple locations, ensuring consistent coverage and continuity of care. You'll collaborate with an interdisciplinary team, document thoroughly in EMR, educate patients and families, and participate in quality and fall-prevention initiatives. Responsibilities Provide physical therapy evaluations and develop individualized treatment plans for seniors in skilled nursing and long-term care settings Float between assigned facilities to provide coverage and maintain continuity of care as patient needs and census change Deliver evidence-based therapeutic interventions to improve mobility, strength, balance, and functional independence Document assessments, goals, treatments, and progress notes accurately and timely in accordance with regulatory and payer requirements Collaborate with an interdisciplinary team, including nursing, physicians, and other therapists, to coordinate patient-centered care Educate patients, families, and caregivers on home exercise programs, safety, and strategies to support functional independence Participate in quality improvement, fall prevention, and rehab outcomes initiatives within assigned facilities Engage in ongoing professional development, mentorship, and clinical training offered by Tender Touch Rehab Services Required Skills Orthopedic and neuromuscular assessment Gait and balance training Therapeutic exercise prescription Manual therapy techniques Use of modalities (e-stim, ultrasound, heat/cold) Care planning in SNF/LTC settings EMR and rehab documentation Interdisciplinary teamwork and communication Discharge planning and caregiver training Knowledge of Medicare/insurance regulations
