Travel PPV Physical Ther/27/HCD268 Physical Therapist (PT)
Location:
Caroline, Talbot, Dorchester, and Queen Anne's counties, MD
Job Type:
Full-Time / Contract
Shift/Schedule:
Monday - Friday, 8:00 AM - 4:30 PM (No on-call or weekend requirements)
Weekly Hours:
36 Hours Guaranteed
Pay Rate:
$49/hr Job Overview We are seeking a dedicated and professional Physical Therapist to join the team. In this role, you will deliver high-quality, patient-centered physical therapy services directly to patients in their homes across Caroline, Talbot, Dorchester, and Queen Anne's counties. You will be responsible for comprehensive care delivery, from initial admissions and evaluations to ongoing follow-up visits and discharges, working closely with a multidisciplinary team. Key Responsibilities •
Home Health Admissions:
Conduct comprehensive home health admission assessments to evaluate patient status, safety, and need for skilled therapy services. •
Evaluations & Care Planning:
Perform thorough physical therapy evaluations and establish individualized plans of care aimed at improving mobility, strength, and functional independence. •
Follow-Up Visits:
Execute routine follow-up visits, providing skilled therapeutic interventions and monitoring patient progress toward goals. •
Discharge Assessments:
Complete accurate and timely discharge assessments and documentation upon the completion of the care plan. •
Care Coordination:
Collaborate effectively with an interdisciplinary healthcare team to ensure coordinated and safe patient-centered care. •
Provider Communication:
Proactively communicate with the patient's primary provider and the care team regarding status changes, progress, or safety concerns. Required Qualifications •
Education:
Graduate of an accredited Physical Therapy program. •
Licensure:
Current and active Physical Therapist license in the state of assignment. •
Certification:
Current CPR certification (BLS - Healthcare Provider). •
Logistics:
Valid driver's license, reliable transportation, and active auto insurance.
