Travel Physical Therapist | Full‑Time | Monday-Friday 8:00 AM-4:30 PM | No Weekends/On‑Call Job Overview We are seeking a dedicated Travel Physical Therapist to provide high‑quality home‑based rehabilitative care across Caroline, Queen Anne, and Talbot Counties . This full‑time role offers a 40‑hour workweek , no weekend or on‑call requirements , and mileage reimbursement between patient visits. Productivity expectation is 30 points per week . Medical, Dental , and Vision Insurance is offered to qualified candidates.

Pay Rate:

$50/hr Top Responsibilities Patient Evaluations — Conduct comprehensive assessments to determine functional status, mobility limitations, and individualized treatment needs. Therapeutic Interventions — Provide strengthening, gait training, balance work, neuromuscular re‑education, and functional mobility training. Care Plan Development — Establish and update treatment plans aligned with physician orders and patient goals. Documentation — Complete timely and accurate charting in HCHB (HomeCare HomeBase). Patient & Caregiver Education — Teach home exercise programs, safety strategies, and functional mobility techniques. Interdisciplinary Collaboration — Communicate with nurses, physicians, and care coordinators to ensure continuity of care. Travel Between Visits — Provide care across multiple counties with mileage reimbursement.
