Position Summary Interim HealthCare is seeking a dedicated and compassionate Physical Therapist (PT) to join our growing Home Health team. In this role, you will provide skilled physical therapy services to patients in their homes, helping them improve mobility, restore function, reduce pain, and achieve the highest level of independence possible. You will work collaboratively with physicians, nurses, therapists, and caregivers to develop individualized treatment plans while providing exceptional clinical care and patient education. Responsibilities Assist physicians and other licensed practitioners in evaluating patient function, performance, and equipment needs. Develop individualized plans of care, including short- and long-term treatment goals, interventions, and recommended frequency and duration of services. Complete comprehensive initial and ongoing patient assessments according to agency and regulatory requirements. Provide skilled physical therapy interventions in accordance with physician orders and professional standards of practice. Monitor patient progress and regularly assess changes in condition to determine if treatment plans require modification. Perform re-evaluations as needed and obtain updated physician orders when appropriate. Educate patients, families, and caregivers on exercises, safety techniques, mobility strategies, and disease management to promote successful outcomes. Collaborate with the interdisciplinary care team to ensure coordinated, patient-centered care. Consult with caregivers and family members regarding patient progress and discharge planning. Complete accurate, timely, and compliant clinical documentation for all visits, treatments, and patient progress. Maintain compliance with all federal, state, agency, and professional practice standards. Qu alifications Graduate of an accredited Physical Therapy program. Current Physical Therapist license in the state of practice. Current CPR certification (or ability to obtain). Valid driver's license, reliable transportation, and proof of auto insurance. At least 2 years of Home Health Experience Strong clinical assessment, communication, and documentation skills. Ability to work independently while collaborating effectively with a multidisciplinary team.
