Job Description

Physical Therapist We are seeking a dedicated Physical Therapist (PT) to join our healthcare team. The Physical Therapist will evaluate, develop, and implement individualized treatment plans to help patients improve mobility, reduce pain, restore function, and enhance overall quality of life. This role works collaboratively with physicians, nursing staff, and interdisciplinary teams to support patients across a variety of care settings.
Key Responsibilities:
Perform comprehensive patient evaluations and assessments to determine functional limitations and treatment goals Develop and implement individualized physical therapy treatment plans Provide therapeutic interventions including exercise, manual therapy, mobility training, and patient education Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes Educate patients and families on treatment goals, home exercise programs, injury prevention, and recovery expectations Maintain accurate and timely clinical documentation in accordance with regulatory and facility requirements Collaborate with physicians, nursing staff, case management, and other rehabilitation professionals Ensure compliance with all safety, quality, and patient care standards
Qualifications:
Doctorate or Master s degree in Physical Therapy from an accredited program Current Physical Therapist licensure (or eligibility) in the applicable state BLS certification preferred or required based on facility guidelines Strong communication, interpersonal, and clinical decision-making skills Ability to work independently and within a multidisciplinary team
Preferred Experience:
Previous experience in acute care, outpatient, inpatient rehabilitation, skilled nursing, or orthopedic settings preferred Experience with EMR documentation systems preferred
