Overview Physical Therapist | Skilled Nursing Facility travel assignment in Skowhegan, Maine. Start date 08/31/2026 for a 13-week contract. Weekly pay range $2,201-$2,296 per week with guaranteed hours of 35 each week.

08:00-16:30, 5 days per week in a skilled nursing facility setting. This travel assignment is conducted through Healthforce Healthcare LLC.

Skilled Nursing Facility (SNF) Extension opportunities available Responsibilities Conduct comprehensive PT evaluations for residents and develop individualized treatment plans targeting mobility, strength, balance, gait, and range of motion. Implement evidence-based interventions to improve functional independence and safety with activities of daily living. Monitor progress, modify plans as needed, and document outcomes in the resident's medical record. Provide patient and family education on exercises, safety strategies, and home programs; collaborate with the interdisciplinary team to coordinate care. Participate in discharge planning, referrals, and transitions of care to support continued progress. Requirements Doctor of Physical Therapy (DPT) degree from an accredited program. Active Physical Therapist license in Maine or eligibility for licensure in Maine. Ability to work in a skilled nursing facility setting and deliver care on a travel assignment. Relevant experience in geriatric rehabilitation or SNF settings preferred. Willingness to travel for a 13-week assignment and adhere to assignment schedule (08:00-16:30, 5 days per week) with 35 guaranteed hours per week. Strong communication and teamwork skills; comfortable documenting care and collaborating with an interdisciplinary team.
