Responsibilities Key Responsibilities Perform comprehensive patient evaluations and develop individualized, goal-directed treatment plans. Regularly reassess treatment plan effectiveness; actively participate in staffing meetings and family conferences. Conduct discharge evaluations; recommend home and community safety modifications; provide referrals and educational resources to support continuity of care. Communicate patient needs, progress, and outcomes clearly to the interdisciplinary team, physicians, patients, and families. Collaborate across disciplines (PT, OT, SLP, nursing, neuropsychology) to coordinate care and optimize patient outcomes. Qualifications Education & Licensure Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program. Current, unrestricted Physical Therapist license in the state of practice.
Certifications & Compliance Current Basic Life Support (BLS) or CPR certification, per state and facility requirements. Willingness to complete required health screenings/exams per location or state policy.
Experience 1+ years of physical therapy experience preferred; new grads with strong neuro rehab clinical rotations will be considered. Demonstrated knowledge of evidence-based rehabilitation techniques for complex neurological conditions (e.g., TBI, SCI, stroke, neurodegenerative disorders).Physical RequirementsAbility to lift up to 50 lbs and safely assist with patient transfers up to 100 lbs using proper body mechanics and available equipment.
Sustained physical stamina, including fine motor skills, visual acuity, and the ability to stand/walk for extended periods; push, pull, bend, stoop, reach, kneel, and position patients as needed.
