Join us at Rivetus Rehab! Take Your Therapist Home As a PT in this role, you will become an integral part of our innovative Take Your Therapist Home Program! What is Take Your Therapist Home? In a world of shorter SNF stays, Take Your Therapist Home (TYTH) is a one-of-a-kind program where clinicians work directly with patients in both the SNF and home health settings, providing continuity of care and a greater impact on patient outcomes. You will primarily work in home health with dedicated time and caseload support for the SNF. Your coverage area will remain localized around the SNF—no more ever-expanding home care coverage areas. Your schedule follows a traditional work week. Rivetus has served the geriatric Post-Acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians. Our unique perspective provides continuity of care across the Skilled Nursing, Home Health, and Outpatient settings.

Facility Location:
Woodward Hills - 39312 N Woodward Ave Bloomfield Hills, MI 48304. This hybrid role provides facility-based therapy and local home care services. Facility services are paid hourly; home care services are paid per visit.

Friday, Full Time General Summary:
In this role, you will provide physical therapy services to patients with physical dysfunction, including assessment, treatment planning, and implementation. You will participate in patient care conferences, screenings, and program development; supervise PTAs and aides as assigned; complete required documentation; and adhere to company policies, procedures, and current clinical practice standards.
Your Responsibilities:
Conduct physical therapy screenings and evaluations to determine patient physical status and develop, implement, and assess treatment plans using strengthening, modalities, and retraining to maximize independence. Provide direct patient care in accordance with productivity standards and the professional and ethical standards of the APTA, using graded activities to restore physical and functional abilities. Check, instruct, assign, and review the work of PTAs and aides as assigned. Maintain accurate documentation to support medical record quality, continuity of care, and reimbursement guidelines. Participate in patient care conferences, CQI activities, and new program development as requested or required. Promote current clinical practice standards through continuing education and in-service programs, including coordinating and/or participating in programs for internal and external groups. Participate in company meetings and additional activities as directed to facilitate effective departmental operations. Adhere to company policies, procedures, and current standards of clinical practice while maintaining a continuous focus on quality patient care.
Required Qualifications:
Successful completion of an accredited Physical Therapy program Licensure as required by the State of Michigan Valid driver's license
Skills and Abilities:
Ability to exercise professional judgment and initiative when analyzing problems and recommending solutions Initiative and flexibility in the work environment Ability to react effectively to the needs of patients and their families Physical ability to bend, position, transfer, and lift patients weighing 80 pounds to over 300 pounds without restriction, utilizing clinically recommended lifting devices
