In this role, you will be responsible for assessing, developing, and implementing personalized treatment plans to help patients regain mobility, reduce pain, and improve overall quality of life. You will work closely with a multidisciplinary team to deliver exceptional patient care across various settings, including outpatient clinics, inpatient hospitals, home health environments, and community care programs. Your expertise in physiotherapy and patient rehabilitation will be vital in supporting patients of all ages, from pediatrics to geriatrics, ensuring they achieve their health and functional goals. Responsibilities Conduct comprehensive patient assessments to determine physical therapy needs, including evaluating physiology, anatomy, and medical history. Develop and implement individualized treatment plans utilizing evidence-based practices such as trigger point therapy, orthopedics, neurology, and geriatrics care. Collaborate effectively with healthcare professionals—including physicians, nurses, occupational therapists—to coordinate patient care and discharge planning. Document all clinical activities accurately following clinical documentation standards and maintain up-to-date electronic health records (EHR) or electronic medical records (EMR) systems. Provide patient education on injury prevention, treatment exercises, and home & community care strategies to promote ongoing recovery outside clinical settings. Manage discharge planning processes by preparing care plans that facilitate smooth transitions from hospital or inpatient settings to outpatient or home health environments. Engage in ongoing professional development to stay current with physiotherapy techniques, medical terminology updates, and new treatment modalities relevant to outpatient rehabilitation centers or inpatient hospital experiences. Experience Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings. Demonstrated expertise working with diverse patient populations including pediatrics, neurology cases, orthopedics, and geriatrics. Strong background in outpatient rehabilitation center experience or inpatient hospital experience is preferred. Familiarity with clinical documentation standards and EMR/EHR systems for accurate patient care documentation. Knowledge of discharge planning procedures and working collaboratively with healthcare teams to develop effective care plans. Prior experience managing patient rehabilitation programs focused on mobility improvement and pain management through physiotherapy techniques such as trigger point therapy. Excellent client communication skills to educate patients about their treatment plans and foster a positive healing environment.
