Job Description

Physical Therapist (On-Call): Neuro Rehabilitation Make a real impact-help someone regain independence while building a career you're proud of. The On-Call Physical Therapist provides skilled physical therapy services to individuals in Hope Network's Neuro Rehabilitation program. In this flexible role, you'll evaluate, treat, and educate individuals recovering from neurological injuries and conditions while collaborating with an interdisciplinary team to improve mobility, function, and quality of life. While this is an on-call position, consistent scheduling opportunities are anticipated during an upcoming maternity leave coverage. What You'll Do Evaluate individuals and develop individualized physical therapy treatment plans Provide hands-on therapy to improve strength, balance, mobility, coordination, and functional independence Design and monitor home exercise programs and mobility interventions Train direct care staff on safe transfers, gait techniques, and therapy recommendations Collaborate with PTAs, rehabilitation staff, physicians, and interdisciplinary team members Complete timely documentation, progress notes, and treatment reports Participate in care planning, discharge planning, family education, and team meetings Monitor therapy equipment and recommend adaptive equipment or mobility devices as needed What We're Looking For Compassionate and patient with a passion for helping others achieve meaningful recovery Strong clinical reasoning and problem-solving skills Excellent communication and collaboration abilities Organized and detail-oriented with strong documentation skills Flexible and adaptable in a dynamic rehabilitation environment
What You Bring Preferred:
Two or more years of experience working with adults with neurological disabilities or traumatic brain injuries (TBI)
Required:
