$56 to $59 per hour Primary Responsibilities:
Perform Physical Therapy admissions, start of care visits, add-on evaluations, reassessments, and discharges Develop individualized plans of care for home health patients Provide therapeutic interventions to improve mobility, strength, endurance, pain management, and functional status Supervise and collaborate with PTAs in patient care delivery Participate in weekly case conferences and monthly staff meetings Document care and patient progress in alignment with home health standards Collaborate with interdisciplinary teams to ensure coordinated patient care Travel within assigned coverage areas across all of Delta County and portions of Dickinson and Menominee Counties (within ~1-hour radius of Escanaba) Qualifications:
Active Physical Therapist license in Michigan required Home Health experience required Experience with Home Care Home Base (HCHB) preferred Strong communication, organization, and patient care skills Wound care or lymphedema certification a plus Ability to follow directions, use GPS, and travel efficiently between visits Why work for Adecco ?
