Adecco Healthcare & Life Sciences is hiring a Travel Physical Therapist in Escanaba, Michigan! This role is on-site / in-person in a home health and hospice setting. Please review the details below and with an updated resume for consideration.

- Friday | 8:00 AM
- 4:30 PM (clinicians self-schedule within office hours)
Weekly Guarantee:
32 hours

Pay:
$56 to $59 per hour
Primary Responsibilities:
- Perform Physical Therapy admissions, start of care visits, add-on evaluations, reassessments, and discharges
- Develop individualized plans of care for home health patients
- Provide therapeutic interventions to improve mobility, strength, endurance, pain management, and functional status
- Supervise and collaborate with PTAs in patient care delivery
- Participate in weekly case conferences and monthly staff meetings
- Document care and patient progress in alignment with home health standards
- Collaborate with interdisciplinary teams to ensure coordinated patient care
- Travel within assigned coverage areas across all of Delta County and portions of Dickinson and Menominee Counties (within ~1-hour radius of Escanaba)
Qualifications:
- Active Physical Therapist license in Michigan required
- Home Health experience required
- Experience with Home Care Home Base (HCHB) preferred
- Strongmunication,anization, and patient care skills
- Wound care or lymphedema certification a plus
- Ability to follow directions, use GPS, and travel efficiently between visits
