Responsibilities Provide physical therapy services to patients in their homes across Wayne, Macomb, and Oakland counties. Develop individualized treatment plans based on patient assessments. Monitor patient progress and adjust treatments as necessary. Maintain accurate and timely documentation of all patient interactions. Collaborate with other healthcare professionals to ensure comprehensive patient care. Requirements Licensed Physical Therapist in the state of Michigan. Ability and willingness to travel to Wayne, Macomb, and Oakland counties. Strong communication and interpersonal skills. Valid driver's license and reliable transportation.
