Renewal Rehab is seeking a Physical Therapist to provide compassionate, evidence-based care for seniors in skilled nursing and post-acute care settings. In this on-site role, you'll evaluate, plan, and deliver individualized treatment focused on mobility, safety, and meaningful functional outcomes. You'll collaborate with an interdisciplinary team, maintain thorough documentation, and educate patients and families. Responsibilities Provide evidence-based physical therapy evaluation and treatment for seniors in skilled nursing and post-acute settings Develop individualized treatment plans focused on functional outcomes, mobility, and fall prevention Document assessments, goals, and progress in accordance with regulatory and facility standards Collaborate closely with an interdisciplinary team, including nursing and rehab professionals, to coordinate care Educate patients, families, and caregivers on exercises, safety, and home programs to support long-term function Participate in quality improvement, clinical programs, and mentorship activities that support best practices Manage a reasonable caseload while maintaining high-quality, patient-centered care and timely documentation Required Skills Geriatric physical therapy Therapeutic exercise prescription Functional mobility and transfer training Balance and fall-prevention interventions Post-acute and long-term care rehab Care planning and documentation (EMR) Interdisciplinary team collaboration Patient and family education Outcome measurement and goal setting Pain management modalities
