Qualifications Physical Therapy License Full Job Description Job Summary:
A Homecare Physical Therapist provides skilled physical therapy services to patients in their homes, helping them recover from injuries, surgeries, or chronic conditions while promoting independence and improving overall mobility. This role focuses on personalized, patient-centered care in a home environment, requiring strong clinical judgment, adaptability, and communication skills.
Key Responsibilities:
Conduct comprehensive in-home evaluations of patients' physical abilities, functional limitations, and living environments Develop and implement individualized treatment plans tailored to patient goals and home settings Provide therapeutic exercises, manual therapy, gait training, and balance activities Educate patients and caregivers on safe mobility techniques, fall prevention, and home exercise programs Monitor patient progress and adjust treatment plans as needed Document patient care accurately and in compliance with healthcare regulations and agency policies Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals Ensure a safe and supportive home environment by recommending adaptive equipment or modifications Travel to patient homes within assigned service areas
Qualifications:
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program Current state licensure as a Physical Therapist Valid driver's license and reliable transportation CPR certification (or willingness to obtain) Prior experience in home health or outpatient care preferred
Skills & Competencies:
Strong clinical reasoning and problem-solving abilities Excellent communication and interpersonal skills Ability to work independently and manage time effectively Adaptability to varying home environments and patient needs Compassionate, patient-focused approach to care
Work Environment:
Patient homes in diverse settings Regular travel required Flexible scheduling, including potential evenings or weekends
Physical Requirements:
