Physical Therapist (PT, School-based)
Compensation Range:
$64-69/hour
Location:
Pontiac, MI; Onsite - School Setting Position Summary The School Physical Therapist will be assessing students' physical development level; providing appropriate treatment to meet individualized student objectives; providing recommendations for program development and student placement; and providing direction to Physical Therapy Assistants. We're searching for a PT who is eager to collaborate with a multidisciplinary team including OTs, SLPs, and other ancillary staff. Shift:
Shift details: School Day Schedule Hours per week: 37.5 hours per week Contract length: School Year Key Duties Interpret physician orders related to professional area of responsibility Assess gross motor skills and therapeutic equipment needs of students Develop, implement, monitor and modify treatment plans based on physician orders and assessment of student as appropriate Collaborates with the school team to develop Individualized Education Programs and Section 504 Plans to address the students' unique needs Consults with teachers, parents, other personnel, and/or outside professionals for the purpose of providing requested information, developing plans for services, and/or making recommendations Coordinates with community agencies including medical, judicial, social service, and mental health services for the purpose of determining appropriate treatments, etc to meet the needs of specific students Participates in meetings, workshops, and seminars as assigned for the purpose of conveying and/or gathering information required to perform functions Prepares a wide variety of written materials (e.g. evaluations, correspondence, memos, treatment plans, reports, etc.) for the purpose of documenting activities, providing written reference, and/or conveying information Complete IEP, MET, Medicaid, and treatment forms/reports Maintain treatment/progress notes and Medicaid documentation per professional standards Train Paraprofessionals/PTAs on various student plans Regular and predictable attendance Requirements Licenses & Certifications
  • Licensed Physical Therapist - State of Michigan
  • Michigan Driver's License
