Contact recruiter for additional details Position Summary We are seeking a skilled and compassionate Physical Therapist (PT) to join a collaborative healthcare team. This position is primarily focused on outpatient physical therapy, with occasional floating to acute care. The Physical Therapist provides patient-centered evaluation, treatment planning, intervention, and follow-up in accordance with physical therapy standards, physician orders, and applicable policies and regulations. The position also provides clinical direction and supervision to Physical Therapist Assistants, aides, and students as appropriate. Essential Responsibilities Evaluate patients using formal and informal testing methods to assess skeletal, neurological, respiratory, and cardiovascular function. Develop individualized, goal-oriented treatment plans based on assessment findings and patient tolerance. Establish treatment frequency and duration in collaboration with patients, families, caregivers, and the clinical team. Submit written treatment plans for physician authorization and coordination. Update plans of care as needed based on changes in patient condition. Provide appropriate supervision and clinical direction to Physical Therapist Assistants, aides, home health aides, and students in accordance with applicable standards. Provide therapeutic interventions designed to facilitate or restore optimal patient function. Educate patients, families, and caregivers regarding diagnoses, treatment options, prognosis, and the rationale for recommended interventions. Develop and provide home exercise and education programs using effective teaching methods. Recommend adaptive and supportive equipment based on patient assessments and assist with obtaining necessary equipment. Document patient responses to treatment and changes in condition in accordance with organizational and regulatory requirements. Complete required daily documentation for billing and compliance purposes. Participate in team conferences and interdisciplinary meetings as appropriate. Coordinate patient care with physicians and other members of the interdisciplinary healthcare team. Function as case manager for assigned patients when applicable. Promote continuous improvement in patient care and participate in program development initiatives. Provide education and professional development to staff, students, and other healthcare providers as appropriate. Promote physical therapy services within the organization and surrounding community. Maintain patient and organizational confidentiality at all times. Demonstrate professionalism, accountability, teamwork, and commitment to high-quality patient care. Perform other duties as assigned. Required Qualifications Bachelor's degree — Required. Current Physical Therapist (PT) license — Required. Ability to meet all applicable state and regulatory licensing requirements. Strong verbal and written communication skills. Strong organizational, problem-solving, and time-management skills. Ability to work independently while effectively collaborating with an interdisciplinary team. Ability to educate patients, caregivers, and other healthcare professionals regarding appropriate treatment techniques. Successful completion of required organizational orientation and new-hire competency requirements. Certifications Physical Therapist (PT) License — Required BLS - Basic Life Support — Highly Preferred; must be obtained within 1 month of hire or transfer Additional Requirements OSHA training is required and must be completed annually in accordance with applicable exposure-control requirements. Ability to work in an environment where exposure to communicable diseases may occur. Ability to correctly use required personal protective equipment. Selected candidates will be required to complete fingerprinting as part of the hiring process. The employer will cover the cost of fingerprinting. Ability to perform essential job functions with reasonable accommodation. Physical & Work Environment This position involves working in a healthcare environment that may include exposure to stressful situations, trauma, grief, death, bloodborne pathogens, and communicable diseases. The role requires the ability to: Sit, stand, and walk for extended periods. Frequently change positions, including stooping, kneeling, or crouching. Move freely throughout the facility with or without an assistive device. Perform activities involving reaching, grasping, handling, fingering, and feeling. Safely use required personal protective equipment. Lift or carry up to 50 pounds occasionally and approximately 20 pounds frequently. Manage multiple tasks while maintaining patient safety and quality of care. Maintain regular and predictable attendance.
