Home Health Physical Therapist:
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function. Adjust treatment plans and techniques as needed to achieve optimal outcomes. Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care. Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices. Document all visits, assessments, and progress notes accurately and in a timely manner. Support patients in maximizing their independence and improving quality of life—wherever they call home. About You Qualifications•What You'll Bring:
Master's degree in Physical Therapy from an accredited program. Current PT license in the state of employment (or eligibility to obtain). CPR certification required. Strong communication, assessment, and problem-solving skills. Ability to work independently in a variety of home environments. Valid driver's license, auto insurance, and reliable transportation.
