What You'll Be Doing As a Physical Therapist, you'll provide individualized care designed to improve strength, balance, mobility, safety, and overall quality of life. You'll work closely with patients and families to create realistic goals and treatment plans that support long-term independence.
Responsibilities include:
Providing in-home evaluations and treatment visits. Completing Start of Care assessments and admissions. Developing individualized plans of care based on patient goals and clinical findings. Delivering evidence-based therapeutic interventions. Supervising and collaborating with Physical Therapist Assistants. Coordinating care with nurses, physicians, and interdisciplinary team members. Monitoring patient progress and adjusting plans of care when needed. Completing documentation and OASIS assessments in accordance with regulatory requirements. What We're Looking For Current Minnesota Physical Therapist License (PT or DPT) Required Home health experience preferred. OASIS experience preferred but not required. Strong clinical judgment and decision-making skills. Excellent communication and relationship-building abilities. Ability to independently manage a territory and caseload.
