Rather than watching the clock until your next patient walks into a clinic, you're moving through your community and managing a caseload of your own. You'll evaluate patients in the exact environment they're trying to safely navigate. You'll see the stairs. You'll see the bathroom setup. You'll see how they transfer from their actual bed. What You'll Be Doing Perform comprehensive in-home Physical Therapy evaluations. Develop individualized treatment plans. Provide hands-on therapy, gait training, balance interventions, strengthening, and exercise progression. Address mobility, transfers, fall prevention, and functional independence. Recommend appropriate equipment and home modifications. Educate patients, families, and caregivers. Monitor progress and modify treatment plans as appropriate. Collaborate with PTAs, nurses, physicians, OTs, and other interdisciplinary clinicians. Complete accurate and timely documentation. Your PT Background Can Translate to Home Health We're interested in Physical Therapists coming from Home Health, Outpatient Orthopedics, Acute Care, Inpatient Rehabilitation, Skilled Nursing, Transitional Care, Senior Living, and other rehabilitation settings. Home health experience is preferred, but it's not the only way to be successful. We're looking for clinicians who are independent, organized, motivated, clinically confident, and comfortable taking ownership of their workday. Required Qualifications Active Minnesota Physical Therapist license PT or DPT Valid driver's license and reliable transportation Strong clinical assessment and reasoning skills Ability to independently manage a field-based schedule and caseload Excellent patient and caregiver communication skills Preferred Qualifications Home health experience OASIS experience Experience with medically complex or geriatric patients Strong interest in one-on-one, patient-centered rehabilitation A Better Schedule Doesn't Have to Mean a Smaller Career You can protect your weekends.
