Your patients are seen during the week. Monday through Friday. 8:00 a.m. to 5:00 p.m. When Friday afternoon arrives, you're done until Monday. No weekend treatment rotation. No holidays. Our territory-based approach allows Physical Therapists to provide care throughout a defined geographic area rather than reporting to the same clinic every morning. Your day happens in the communities you're serving. You'll travel directly between patients, manage your schedule, and provide rehabilitation in the environment where your patients actually need to function. Your Patients Aren't Trying to Get Better at Physical Therapy. They're Trying to Get Better at Life. That's one of the biggest differences in home health. A patient isn't working on stairs because it's part of today's treatment plan. They need to reach their bedroom. They aren't practicing transfers simply because it's a functional goal. They want to independently get out of their favorite chair. They aren't working on balance because you need another measurable outcome. They want to walk outside without being afraid they'll fall. Home health puts those goals directly in front of you. What You'll Be Doing Complete comprehensive Physical Therapy evaluations in patient homes. Develop individualized plans of care based on functional needs and patient goals. Provide therapeutic interventions addressing strength, balance, gait, mobility, and safety. Progress exercises and functional activities based on patient response. Identify fall risks and barriers to independence within the actual home environment. Recommend equipment and appropriate safety modifications. Educate patients, families, and caregivers. Collaborate with PTAs, nurses, physicians, OTs, and other interdisciplinary professionals. Monitor patient progress and adjust treatment plans when clinically appropriate. Complete accurate and timely clinical documentation. Already a Great PT? We Can Teach You Home Health. Previous home health experience is valuable, but it isn't the only background we're interested in. Physical Therapists coming from Outpatient Orthopedics, Acute Care, Inpatient Rehabilitation, Skilled Nursing, Transitional Care, Senior Living, and other rehabilitation environments can bring highly transferable skills to home-based care. We're looking for someone who is clinically confident, organized, comfortable working independently, and passionate about helping patients regain meaningful function. Qualifications Active Minnesota Physical Therapist license PT or DPT Valid driver's license and reliable transportation Strong clinical assessment and decision-making skills Ability to independently manage a field-based schedule Excellent patient and caregiver communication skills Home health experience preferred OASIS experience preferred Build the PT Career You Want Without Building Your Life Around Work Strong compensation shouldn't automatically require nights, weekends, or holidays.
