You'll evaluate patients where they live, identify barriers to independence, and create treatment plans that have a direct impact on safety, mobility, and quality of life. Instead of focusing solely on impairments, you'll help patients return to the activities that matter most to them. Responsibilities Provide in-home physical therapy evaluations and treatment visits. Complete admissions and start-of-care assessments. Develop individualized treatment plans based on patient goals. Deliver therapeutic interventions focused on strength, balance, mobility, and functional independence. Supervise and collaborate with Physical Therapist Assistants. Coordinate care with nurses, therapists, physicians, and other members of the interdisciplinary team. Monitor patient progress and adjust plans of care as needed. Complete documentation, including OASIS when applicable. Qualifications Current Minnesota Physical Therapist license. Home health experience preferred. OASIS experience preferred. Ability to independently manage a territory and caseload. Valid driver's license and reliable transportation. Strong clinical judgment and patient communication skills.
