Provides in-home evaluation, direction, and application of Physical Therapy services to relieve pain, develop or restore function, and/or maintain maximum physical performance of patient.
Education:
Required:
Graduate of approved program in Physical Therapy as referenced in Conditions of Participation 54 FR 33354 (@484.4), or Two years experience as Physical Therapist and satisfactory grade on proficiency exam conducted, approved, or sponsored by U.S. Public Health Service. (N/A if licensed or sought qualification as PT after December 31, 1977).
