Job Overview We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. In this role, you will deliver high-quality rehabilitative care to seniors in a community for independent, assisted and memory care residents. Your expertise will help patients regain mobility, reduce pain, and improve their overall quality of life through personalized treatment plans. This position offers flexibility and the opportunity to make a meaningful impact in an outpatient environment. Duties Assess patients' physical conditions through comprehensive evaluations, including anatomy and physiology knowledge, to develop effective care plans. Implement treatment plans utilizing techniques such as therapeutic exercises and mobility training. Collaborate with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate patient care. Document patient progress accurately in electronic health records system point of service documentation. Educate patients on treatment goals, home exercise programs. Manage planning processes by assessing patient needs for continued therapy. Skills Strong understanding of medical terminology and clinical documentation standards essential for accurate record-keeping per Medicare guidelines Proven experience in outpatient environments with a focus on orthopedics, neurology and, geriatrics. Ability to develop individualized treatment plans that incorporate techniques tailored to patient needs. Excellent client communication skills to motivate and educate patients throughout their treatment journey. Experience working with geriatric patients highly desirable. Familiarity with inpatient care procedures and acute care experience enhances your ability to adapt quickly across different clinical scenarios.
