Job Description

Liberty Health seeks a Physical Therapist to deliver person-centered rehabilitation services within innovative behavioral health and human services programs. The PT will evaluate functional mobility, develop evidence-based treatment plans, and provide individualized interventions to support independence and community integration. Responsibilities include collaborating with interdisciplinary teams, documenting in EHR systems, educating patients and caregivers, and tracking functional outcomes. This role supports public-sector partners with data-driven, outcomes-focused care across diverse populations in a collaborative, accountable culture. Responsibilities Conduct comprehensive PT evaluations and functional mobility assessments Develop and implement evidence-based, person-centered treatment plans Provide therapeutic exercises, gait, balance, and neuromuscular re-education Collaborate with interdisciplinary behavioral health and human services teams Document assessments, plans, and progress in EHR systems accurately and timely Educate patients, families, and caregivers on home programs and safety Monitor and report functional outcomes and contribute to quality improvement initiatives Support public-sector partners in implementing data-driven, community-based care Required Skills Physical therapy evaluation Therapeutic exercise design Neuromuscular re-education Gait and balance training Manual therapy techniques Functional mobility training Interdisciplinary team collaboration Electronic health record (EHR) documentation Patient and caregiver education Outcome measurement and reporting
