$50/hr Position Overview One of our Clients are seeking a dedicated Physical Therapist (PT) to provide high‑quality home health services to patients across Union County or East Mecklenburg County. This role offers a stable weekday schedule, guaranteed weekly hours, and mileage reimbursement. Key Responsibilities Each item begins with a Guided Link so you can expand any section later. Home Health PT Visits — Deliver skilled physical therapy services in patient homes, including therapeutic exercises, mobility training, and functional assessments. Start of Care (SOC) Evaluations — Complete comprehensive SOC assessments and establish individualized treatment plans. Recertifications — Conduct timely recert evaluations to update goals and progress. Routine Visits — Provide ongoing therapy interventions aligned with patient goals and physician orders. Discharges — Perform discharge assessments and ensure safe transition from home health services. Documentation — Complete accurate, timely charting in accordance with agency and regulatory standards. Required Qualifications Current NC PT License — Active and unrestricted Home Health Experience — Preferred but not required Clinical Competency — Strong assessment, mobility training, and therapeutic intervention skills Valid driver's license and reliable transportation
