Job Description
Liberty Healthcare Management seeks a compassionate Physical Therapist to deliver high-quality, evidence-based rehab services to seniors across skilled nursing, senior living, and home care settings. In this full-time role, you will perform comprehensive evaluations, create and update individualized treatment plans, and focus on restoring function, mobility, and safety. You'll collaborate with an interdisciplinary team, educate patients and families, and document outcomes in accordance with regulatory standards. Responsibilities Provide evidence-based physical therapy evaluations and treatments for senior and rehab patients across post-acute and home care settings Develop, implement, and regularly update individualized care plans focused on functional recovery, mobility, and safety Collaborate closely with physicians, nursing, rehab, and interdisciplinary teams to coordinate patient-centered care Educate patients, families, and caregivers on home exercise programs, adaptive strategies, and fall prevention Accurately document assessments, progress notes, and outcomes in the EMR in accordance with regulatory and payer requirements Participate in quality improvement, outcomes tracking, and best-practice initiatives to enhance patient care Supervise and mentor PTAs and support staff, promoting professional growth and adherence to clinical standards Communicate effectively with families and care teams, demonstrating compassion, respect, and professionalism Contribute to discharge planning, recommending equipment and services to support safe transitions of care Engage in ongoing professional development and in-service training offered by Liberty Healthcare Management Required Skills Physical therapy evaluation and treatment planning Geriatric and post-acute rehabilitation Manual therapy techniques Therapeutic exercise and neuromuscular re-education Balance, gait, and fall-prevention training Use of assistive devices and durable medical equipment EMR documentation and Medicare-compliant billing Interdisciplinary care coordination Patient and caregiver education Outcomes measurement and quality improvement
