Liberty Health is seeking a compassionate Travel Physical Therapist to deliver high-quality, patient-centered rehabilitation services across diverse care settings. In this role, you will assess, plan, and implement evidence-based treatment programs to restore function, improve mobility, and promote independence for clients of varied ages and diagnoses. You will collaborate closely with Case Managers and interdisciplinary teams, provide patient and caregiver education, document progress, and adapt plans of care to support optimal outcomes. Responsibilities Perform comprehensive physical therapy evaluations and develop individualized plans of care Deliver evidence-based therapeutic interventions to improve strength, mobility, and functional independence Collaborate with Case Managers, physicians, and interdisciplinary teams to coordinate patient-centered care Educate patients and caregivers on home exercise programs, safety, and functional strategies Document assessments, goals, treatments, and progress notes in the electronic medical record Adjust treatment plans based on patient response and evolving clinical needs Ensure compliance with federal, state, and facility regulations and professional practice standards Participate in quality improvement initiatives and contribute to best-practice development Provide coverage across multiple facilities/regions as a traveling clinician, adapting to new environments Mentor or guide support staff and assist with orientation at new assignments Required Skills Physical therapy evaluation and treatment planning Orthopedic and neurologic rehabilitation Gait training and balance assessment Manual therapy techniques Therapeutic exercise prescription Functional mobility and transfer training Wound care and pain management modalities (as applicable) Electronic medical record (EMR) documentation Patient and caregiver education Interdisciplinary team collaboration
