Job Summary We are seeking a compassionate and skilled Physical Therapist to join our Home Care team in Manchester, NH. The Physical Therapist provides in-home physical therapy services to patients of all ages, develops individualized treatment plans, evaluates patient progress, and collaborates with physicians and interdisciplinary care teams to deliver high-quality patient care. This is a full-time, day shift position that includes travel throughout southern New Hampshire. Duties & Responsibilities Perform comprehensive physical therapy evaluations in patients' homes. Develop, implement, and modify individualized treatment plans. Reassess patient progress and update care plans as needed. Coordinate care with physicians, nurses, caregivers, and other healthcare professionals. Educate patients and families on treatment plans, exercises, mobility, and safety. Document evaluations, treatments, and patient progress accurately in the electronic medical record. Supervise Physical Therapist Assistants (PTAs) in accordance with New Hampshire regulations and agency policies. Maintain compliance with organizational policies, safety protocols, infection control standards, and ethical guidelines. Participate in quality improvement initiatives and interdisciplinary team meetings. Travel throughout assigned service areas to provide home health physical therapy services. Participate in a rotating weekend schedule. Requirements Graduate of an accredited Physical Therapy (APTA) program. Current New Hampshire Physical Therapist license . Current BLS certification . Valid driver's license, reliable transportation, and proof of auto insurance . Minimum 1 year of physical therapy experience . Basic computer and electronic documentation skills. Ability to travel throughout southern New Hampshire and participate in a rotating weekend schedule . AL01
