About the Job As a Physical Therapist in our Homecare Therapy Department, you will deliver high‑quality, patient‑centered rehabilitation services directly in the home setting. You'll evaluate functional needs, develop individualized treatment plans, and help patients regain strength, mobility, and independence. This role blends clinical expertise with meaningful relationship‑building and the autonomy to make a real impact in patients' lives. What You'll Do Develop individualized therapy interventions to restore function, reduce pain, and promote optimal health. Evaluate patient progress through observation, measurable outcomes, and clinical judgment; adjust plans as needed. Collaborate with physicians and the homecare team to determine and refine treatment plans. Complete patient admissions , including review of medical records and initial assessments. Design and teach home exercise programs to patients, families, and caregivers to support continued progress after discharge. Document measurable goals and clinical services in the electronic health record within required timelines. Communicate effectively with team members and the Homecare Team Manager regarding patient needs and care plans. Participate in interdisciplinary care conferences , staff meetings, and mandatory in‑services. Perform timely therapy reassessments using objective, measurable data. Support daily operations by traveling within the agency's territory in all seasons and accepting visits as needed. Provide excellent customer service and contribute to a positive team culture. Supervise Physical Therapist Assistants (PTAs) in accordance with scope of practice and patient needs.
