•Job Summary We are seeking a dedicated and skilled Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our healthcare team. The ideal candidate will possess a strong foundation in physiotherapy principles, with experience in patient care across diverse settings such as outpatient clinics, inpatient hospitals, and home health environments. This role involves developing personalized treatment plans, collaborating with multidisciplinary teams, and delivering comprehensive rehabilitative services to improve patient mobility and quality of life. The successful applicant will demonstrate excellent communication skills, a thorough understanding of anatomy and physiology, and a commitment to adhering to clinical documentation standards. Responsibilities Assess patients' physical conditions through detailed evaluations, including mobility, strength, and functional capacity assessments. Develop and implement individualized treatment plans tailored to each patient's needs, goals, and medical history. Provide hands-on therapy techniques such as trigger point therapy, manual therapy, and therapeutic exercises to facilitate recovery. Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes and ensure safety. Collaborate effectively with healthcare professionals including physicians, nurses, and occupational therapists to coordinate care plans. Document all patient interactions accurately within electronic health records (EHR) systems in compliance with clinical documentation standards. Assist in discharge planning by evaluating patient needs for ongoing therapy or community-based care post-treatment. Engage with patients across various age groups—including geriatric, pediatric, neurological, and orthopedic populations—to deliver targeted rehabilitation services. Educate patients and their families on treatment procedures, home exercises, and community resources to support continued recovery outside clinical settings. Requirements Valid licensure or certification as a licensed Physical Therapist or Physical Therapist Assistant in the relevant jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospital settings, or home health environments. Strong knowledge of anatomy, physiology, medical terminology, and physiotherapy techniques relevant to diverse patient populations. Experience with outpatient rehabilitation center operations and inpatient hospital workflows is preferred. Familiarity with electronic health records (EHR) or electronic medical records (EMR) systems for accurate documentation management. Ability to develop effective treatment plans based on comprehensive assessments and clinical observations. Excellent client communication skills with the ability to motivate patients and foster a positive therapeutic environment. Knowledge of discharge planning processes and working with geriatric patients or individuals requiring community-based care is advantageous. Prior experience in specialties such as orthopedics, neurology, pediatrics, or trigger point therapy enhances suitability for this role.
