The ideal candidate will be responsible for evaluating and treating patients, helping improve their mobility and quality of life. The Physical Therapist will work in diverse settings such as day programs and in client homes providing comprehensive care to patients across different age groups, including geriatrics, and those with developmental disabilities. Duties Conduct thorough evaluations of patients to determine their physical therapy needs. Develop individualized treatment plans based on patient assessments and goals. Implement therapeutic interventions, including exercises for strengthening, balance, and mobility. Monitor patient progress and adjust treatment plans accordingly to ensure optimal outcomes. Educate patients and their families on treatment plans, exercises, and self-care strategies. Maintain accurate documentation of patient evaluations, treatments, and progress in compliance with healthcare regulations as well as company documentation requirements

Valid state licensure as a Physical Therapist. Personal liability insurance is required CPR certification is required A means to properly travel between homes and locations. All state onboarding requirements must be met Experience in various settings including home health is preferred. Previous experience working with geriatric populations and those with developmental disabilities Strong communication skills to effectively interact with patients and families as well as effectiveness with caregiver training.
