Full-Time Admissions Physical Therapist (PT) - Home Health Visits Burlington County, NJ Are you an experienced Home Health Physical Therapist looking for a role that offers flexibility, independence, and the opportunity to make a meaningful impact? BAYADA Home Health Care is seeking a Full-Time Admissions Physical Therapist (PT) to provide OASIS Start of Care (SOC) visits for adult and geriatric clients throughout Burlington County, NJ . In this specialized admissions role, you'll focus on conducting comprehensive evaluations, completing Start of Care admissions, and establishing personalized care plans that help patients safely recover and thrive in the comfort of their own homes.

Responsibilities:
Complete OASIS Start of Care (SOC) admissions visits for home health patients throughout Burlington County. Perform comprehensive physical therapy evaluations, including assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities. Develop individualized treatment plans based on patient needs and physician orders. Collaborate with nursing and interdisciplinary team members to coordinate comprehensive patient care. Educate patients, families, and caregivers to promote independence, safety, and optimal outcomes. Accurately document clinical findings, assessments, and services using electronic medical records (EMR) and tablet technology. Ensure timely and accurate completion of OASIS documentation and admission requirements. Continuously evaluate patient needs and make appropriate recommendations for the ongoing plan of care.
Qualifications:
Current New Jersey Physical Therapist (PT) license. Minimum of one year of recent experience as a Physical Therapist. Prior home health experience required. OASIS experience required. HCHB experience strongly preferred. Strong clinical assessment, communication, organizational, and time-management skills. Ability to work independently and effectively manage a flexible schedule. Additional Information:
