Clinical Supervisor / Director of Rehabilitation About the Role. We are seeking a compassionate, autonomous, and detail‑oriented Physical Therapist (PT) to provide evidence‑based rehabilitative services to clients in their homes across New Jersey. The PT will conduct evaluations, develop and implement individualized treatment plans, and collaborate with an interdisciplinary team to help clients maximize safety, independence, and quality of life. Key Responsibilities Duties Assess patients' physical conditions by reviewing medical histories and conducting comprehensive evaluations involving anatomy and physiology knowledge. Skills Strong knowledge of anatomy , physiology , and medical terminology relevant to physical therapy practice. Proven experience in diverse settings such as acute care , home health , hospital , clinic , or outpatient environments. Expertise in developing and implementing effective care plans for various populations, including geriatrics , pediatrics , and orthopedic patients. Proficiency with EMR systems for documentation and patient record management. Skilled in specialized therapies such as trigger point therapy and other manual techniques. Excellent communication skills for educating patients, families, and healthcare team members. Ability to work independently or as part of a multidisciplinary team to deliver comprehensive patient-centered care. Develop individualized care plans incorporating discharge planning and treatment strategies aligned with patient goals. Administer therapeutic interventions such as trigger point therapy , manual therapy, exercise programs, and other physiotherapy techniques. Monitor patient progress and adjust treatment plans accordingly to optimize recovery outcomes. Collaborate with multidisciplinary teams in inpatient, outpatient, hospital, or home & community care settings to ensure continuity of care. Document all patient interactions accurately using EMR systems and maintain detailed records of treatment sessions and progress notes. Educate patients and their families on injury prevention, mobility exercises, and post-treatment care to support long-term health management. Assist with discharge planning by evaluating patient needs for ongoing therapy or community resources post-treatment. Stay current with advancements in orthopedics , geriatrics , pediatrics , and other specialties to provide the highest standard of care. Qualifications Required Active New Jersey PT license (in good standing) issued by the NJ State Board of Physical Therapy Examiners. Valid driver's license, reliable transportation, and proof of auto insurance. Ability to pass all pre‑employment screening (e.g., background check, health clearance, TB screening) per NJ/state and agency requirements.
