Physical Therapist (PT) - Home Health | Atlantic County, NJ | Part-Time & PRN Stern at Home Therapy is expanding into Atlantic County, NJ through a new community-based care contract and is hiring Part-Time and PRN Physical Therapists to help develop this growing service area. This is an opportunity to join a new market at an early stage, build a caseload throughout Atlantic County, and provide one-on-one care with the flexibility and clinical autonomy of home health.
What You'll Do:
As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population in their homes throughout Atlantic County. Because this is a newly developing coverage area, you will work closely with our clinical and administrative teams as referrals and patient volume continue to grow.
Responsibilities Include:
Perform physical therapy evaluations and develop individualized plans of care. Provide one-on-one physical therapy treatments in patients' homes. Help patients improve mobility, strength, balance, endurance, safety, and functional independence. Address gait training, transfers, fall prevention, pain management, and home exercise programs. Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate. Complete accurate and timely clinical documentation. Communicate with patients, families, physicians, and members of the interdisciplinary care team. Coordinate with our administrative team regarding scheduling, referrals, and patient logistics. Build and manage your caseload based on your availability and preferred coverage area within Atlantic County.
Compensation:
Part-time and PRN positions offer fixed per-visit compensation and do not use a tiered volume model. Part-Time W-2 — Minimum 10
Patients:
$55-$65 per follow-up visit. $75 per 4-unit evaluation. Part-Time 1099 — Minimum 10
Patients:
$72 per follow-up visit. $85 per 4-unit evaluation. PRN — Minimum 5
Patients:
$72 per follow-up visit. $85 per evaluation.

Qualifications:
Active Physical Therapist license in New Jersey required. Graduation from an accredited Physical Therapy program. Reliable transportation required. Strong clinical, communication, documentation, and time-management skills. Ability to work independently in a home-based setting. Home health or geriatric experience is preferred but not required.
