Now Hiring - Physical Therapist PRN The Physical Therapist is responsible for, but not limited to: Provide compassionate, patient-centered care in the home, creating a positive and supportive experience during each visit Conduct in-home assessments to evaluate patients' mobility, functional abilities, pain levels, and overall safety Develop individualized treatment plans aimed at improving ADLs, strength, balance, and overall independence Deliver hands-on therapy and interventions in the patient's home using evidence-based techniques and appropriate assistive devices Educate patients, families, and caregivers on exercises, mobility techniques, and safe practices to support progress between visits Track and evaluate patient outcomes, adjusting plans of care as needed to ensure the most effective results Collaborate with interdisciplinary team members to coordinate care and support smooth transitions between levels of service Document all visits, treatments, and progress in compliance with agency standards and home care regulations Maintain reliable and timely attendance to ensure continuity of care and a strong patient-provider relationship

Physical Therapy Professional Experience:
