As aPhysical Therapist, you will be an integral part of our interdisciplinary team, playing a vital role in helping our residents achieve their highest level of physical function and independence. About the
Role:
As a Physical Therapist at Mountainside Nursing Home, you will be responsible for: Evaluating and treating residents with a variety of physical impairments, including those with musculoskeletal, neurological, and cardiovascular conditions Designing and implementing individualized treatment plans to promote resident independence, function, and mobility Conducting daily evaluations and treatments, as well as providing education and training to residents, family members, and caregivers Collaborating with other healthcare professionals, including physicians, occupational therapists, and speech therapists, to provide comprehensive care Maintaining accurate and timely documentation of treatment plans, progress notes, and resident data
Additional Responsibilities:
Participate in care plan meetings and interdisciplinary team conferences Develop and maintain effective working relationships with residents, families, and staff members Stay current with best practices and standards of physical therapy practice Participate in quality improvement initiatives and peer review processes
Requirements:
Graduate degree in Physical Therapy from an accredited program Minimum 1 years of experience in physical therapy, with a strong desire to work in a long-term care setting Current licensure in the state of [State] as a Physical Therapist Ability to lift, transfer, and support residents with varying weights and abilities
