Responsibilities:
Assess patients in their homes and develop an appropriate treatment plan with measurable goals and time frames Provide PT treatments to patients in the comfort of their homes Prepare and submit clinical and progress summaries based on identified and attained goals Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient
Qualifications:
Bachelor's Degree in PT or higher from an accredited physical therapy program Valid NJ state license to practice PT and NJ registration
