Job Description

Job Overview Join our dynamic healthcare team as a Physical Therapist, where you will play a vital role in enhancing the independence and quality of life for our geriatric patients receiving care in their homes. This is a flexible position that operates out of Monmouth County and Middlesex County in NJ. Who We Are Elite Mobility Home Therapy provides personalized, one-on-one physical therapy in the comfort of our patients' homes. Our model prioritizes quality care, strong communication, and therapist autonomy. We work closely with physicians, specialists, and care teams to deliver coordinated, patient-centered treatment without the burnout of high-volume clinics. Responsibilities Provide one-on-one in-home physical therapy Evaluate patients and develop individualized plans of care Communicate and collaborate with referring physicians, specialists, and care teams Build strong relationships with patients and families to support optimal outcomes Coordinate care and maintain continuity between patients and providers Complete documentation accurately and in a timely manner Represent Elite Mobility Home Therapy with professionalism and compassion Who You Are Licensed Physical Therapist (PT) in good standing Strong communication and interpersonal skills Comfortable working independently in a home-based setting Patient-focused and relationship-driven Valid driver's license and access to a reliable vehicle Home health experience is a plus, but not required

Pay:

$80.00 - $90.00 per hour
