The Physical Therapist is responsible for direct patient services, including evaluation and treatment, participation in in-service programs, recordkeeping, and documentation and provides consultation to staff and family when necessary.Evaluation:
Performs pre-entry evaluation, initial evaluation, and re-evaluations to assess child's development level, and identify handicapping delays and disorders. Establishes long and short-term goals for each child.Treatment:
Plans and administers individual physical potential and development. Modifies therapy program as needed.
Written Reports:
Provide written reports of all evaluation, re-evaluations, progress reports, daily file notes, and annual reviews as scheduled.Qualifications:
- Must possess a minimum of a B.S. in Physical Therapy from an accredited college or university.
- Must be licensed and currently registered in New York State as a Physical Therapist or possess a limited permit to work under a Licensed Therapist.
- Excellent communication and interpersonal skills for effective interaction.
- Ability to work independently and as part of a multidisciplinary team.
- Strong organizational skills for case management and documentation.
