Job Objective:

Responsible for evaluating and treating of Short Term Rehab and long term care residents with weekend rotation

Key Responsibilities:

Evaluate and treat short term rehab and long term care residents. Documentation in accordance with JCAHO, third party payers, and the Thompson Health System. Program development, out come studies, and projects as required

Qualifications:

Valid NYS licensure to practice Physical Therapy Degree in Physical Therapy from CAPTE approved program, DPT preferred Prior experience in long term care and short term rehab setting preferred Experience with EMR Team player and self-motivated

Pay Rate:
