Medical, Dental, and Vision insurance Paid Time Off Daily Pay 401k Direct Deposit We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team in a skilled nursing facility. The Physical Therapist will evaluate residents and develop individualized treatment plans to improve mobility, strength, and overall functional independence while enhancing quality of life.
Key Responsibilities:
Evaluate residents and develop individualized physical therapy treatment plans Provide therapeutic exercises, manual therapy, and mobility training Monitor and document resident progress and adjust care plans as needed Collaborate with interdisciplinary team members to ensure comprehensive care Educate residents and families on treatment plans and at-home exercises Ensure compliance with all state, federal, and facility regulations Maintain accurate and timely documentation in accordance with facility policies Participate in care plan meetings and team discussions
Qualifications:
Licensed Physical Therapist in the state of [State] Graduate of an accredited Physical Therapy program Previous experience in a skilled nursing facility or rehab setting preferred Strong clinical, communication, and documentation skills Ability to work independently and as part of a team Compassionate and patient-focused approach to care
