Job Description

Job Summary Join our dynamic healthcare team as a Home Care Physical Therapist on a per diem, contract basis! In this vital role, you will deliver expert physicaltherapy services directly in patients' homes and community settings, empowering individuals to regain mobility, reduce pain, and enhance their quality of life. Your expertise will help develop personalized care plans, facilitate discharge planning, and coordinate with multidisciplinary teams to ensure seamless patient care across inpatient, outpatient, and home health environments. This position offers flexibility while making a meaningful difference in patients' lives through dedicated, compassionate therapy. Duties Assess patients' physical conditions by reviewing medical histories and conducting thorough evaluations of physiology, anatomy, and functional mobility. Develop and implement tailored care plans that incorporate physiotherapy techniques such as trigger point therapy, orthopedics, geriatrics, pediatrics, and outpatient rehabilitation. Deliver hands-on therapy sessions focusing on improving strength, flexibility, balance, and overall mobility. Coordinate discharge planning by collaborating with physicians, caregivers, and other healthcare professionals to ensure smooth transitions from hospital or inpatient settings to home or community care. Document patient progress accurately using electronic medical records (EMR systems), maintaining detailed records of treatment plans, interventions, and outcomes. Educate patients and their families on injury prevention, proper body mechanics, and home exercise programs to promote ongoing recovery. Monitor patient responses to therapy and adjust treatment strategies accordingly to optimize results. Experience Proven experience in acute care hospital settings with familiarity in inpatient rehabilitation or outpatient clinics Strong background in home & community care environments with demonstrated ability to manage diverse patient populations including geriatrics and pediatrics. Knowledge of medical terminology, physiology, and anatomy essential for accurate assessment and effective treatment planning. Hands-on expertise in physiotherapy techniques such as trigger point therapy and orthopedic rehabilitation. Familiarity with discharge planning processes across hospital and home health settings. Excellent patient care skills combined with the ability to work independently in a flexible per diem capacity while maintaining high standards of professionalism.
