Job Description
Overview Join our dynamic healthcare team as a dedicated Physical Therapist, where your expertise will transform patient recovery journeys and enhance quality of life. In this vital role, you will develop personalized care plans, deliver targeted physiotherapy interventions, and collaborate closely with multidisciplinary teams to ensure optimal patient outcomes. Your passion for helping others and strong clinical skills will drive impactful rehabilitation programs across diverse settings, including outpatient clinics, hospitals, and home health environments. Duties Assess patients' physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to identify functional limitations and develop tailored treatment strategies. Design and implement effective care plans incorporating therapies such as trigger point therapy, orthopedics, neurology, pediatrics, geriatrics, and inpatient rehabilitation techniques. Deliver hands-on physiotherapy interventions to improve mobility, strength, flexibility, and pain management for patients of all ages. Conduct discharge planning and coordinate with healthcare teams to ensure seamless transitions from acute care to outpatient or community settings. Maintain accurate and thorough clinical documentation adhering to standards for medical terminology and electronic health records (EHR) management. Educate patients and caregivers on injury prevention, exercise routines, post-treatment care, and community resources to support ongoing recovery at home or in the community. Monitor patient progress regularly, adjusting treatment plans as needed to optimize recovery outcomes while ensuring compliance with clinical documentation standards. Experience Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or inpatient settings with a focus on orthopedics, neurology, pediatrics, or geriatrics. Demonstrated expertise in discharge planning, acute care experience, and working with diverse patient populations including geriatric patients and those requiring home & community care. Familiarity with outpatient rehabilitation center operations and outpatient clinic experience is highly desirable. Strong knowledge of physiology, anatomy, medical terminology, and trigger point therapy techniques essential for effective patient care. Experience managing electronic health records (EHR) systems or electronic medical records (EMR) systems in compliance with clinical documentation standards. Ability to work collaboratively within multidisciplinary teams in inpatient hospital settings or outpatient clinics while maintaining high standards of patient care. Prior exposure to home health services or outpatient rehabilitation environments enhances your capacity to deliver comprehensive physiotherapy services across various care settings.
