This position offers an exciting opportunity to work across diverse settings—including outpatient clinics, inpatient hospitals, home health environments, and rehabilitation centers—delivering personalized care that makes a real difference. You will develop comprehensive treatment plans, collaborate with multidisciplinary teams, and empower patients on their journey to recovery. Your expertise in physiotherapy and dedication to patient care will help improve quality of life for individuals of all ages, from pediatrics to geriatrics. Responsibilities Assess patients' physical conditions by evaluating their mobility, strength, and functional abilities using physiotherapy techniques. Develop and implement customized treatment plans tailored to each patient's needs, goals, and medical history. Conduct therapy sessions utilizing modalities such as trigger point therapy, manual therapy, and exercise programs to promote healing and improve function. Manage discharge planning processes and coordinate care with healthcare professionals to ensure seamless patient transitions across settings. Maintain accurate clinical documentation in accordance with medical terminology standards and electronic health records (EHR) or electronic medical records (EMR) systems. Collaborate closely with physicians, nurses, occupational therapists, and other healthcare providers to optimize patient outcomes. Provide patient education on injury prevention, rehabilitation exercises, and home care routines to foster independence and long-term wellness. Work with diverse populations including geriatric patients, pediatrics, neurology cases, orthopedics, and those requiring outpatient or inpatient rehabilitation services. Experience Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health environments. Demonstrated expertise in treatment plan development for various conditions such as orthopedics, neurology, geriatrics, pediatrics, and post-surgical rehabilitation. Familiarity with acute care settings and inpatient experience is highly desirable. Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards necessary for effective patient care. Experience working with electronic health records (EHR) systems such as EMR platforms for documentation and patient management. Ability to communicate effectively with clients of all ages while demonstrating compassion and professionalism in patient interactions. Knowledge of discharge planning processes and working collaboratively with healthcare teams to ensure comprehensive care delivery.
