New licensees are welcome! We are seeking a Physical Therapist (PT) to join our Clinical Services Team. We have a full-time opportunity, which will provide coverage across two regions of our agency - within Kingston (Ulster County) and the Carmel area in Putnam County. And we have a part-time option, for two to three days per week, serving the Kingston, NY area. The PT is responsible for carrying out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. Specific responsibilities include, but are not limited to:
- Providing direct patient care, evaluating functional needs and outcomes, and consulting with other specialists. Reviewing patients' functional needs and adjusting care plans to ensure optimal patient outcomes.
- Designing and implementing a plan of care for patients based on a thorough assessment of potential development.
- Directing the assistance of support staff to achieve patients' functional goals.
- Communicating effectively with other professional and support staff to achieve positive patient outcomes.
- Responsible to obtain scripts and perform evaluations upon receipt of referral prescriptions.
- Documenting evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.
- Becoming familiar with the regulations, policies and procedures of the Article 16A Clinic.
- Remain informed and be able to demonstrate best practices in the field of physical therapy The ideal candidate will have experience working with individuals with disabilities, and familiarity of OPWDD regulations and policies.
Some travel within the Kingston area is required.
Job Requirements:
Master's Degree in Physical Therapy and New York State license required. Experience with documentation in an Electronic Health Record preferred. Knowledge and skills to care for the physical and developmental needs of patient. Ability to remain calm in emergency or crisis situations. Experience providing support to people with disabilities is a plus.
